Terms & Conditions

These terms and conditions are incorporated into the agreement between the member and management of Pure Med Therapy and shall be applicable to all services. Applicants are required to provide a valid identification document along with this application form (Passport / Driving License). When services are administered online or communication is over phone call or text, there is always a chance of a security breach of data. Pure Med Therapy will make every attempt to keep your data safe, but please understand that this is a risk with any online platform. Advance booking is required for each appointment. We recommend you to check in at least 15 minutes prior to your appointment. All efforts will be made to accommodate members arriving later than their pre-booked appointment. However, we regret that this may not always be possible due to the pre-booked appointments of other customers. Consequently, the duration of the session of late arrivals may be reduced accordingly. Cancellation of membership is permitted in exceptional cases. If the cancellation is permitted, the used session will be charged at normal session rates and a cancellation fee may also be applied. The management reserves the right to accept or reject the cancellation of membership. Pure Med Therapy shall not be responsible or held liable for safekeeping, loss, theft or damage of any member’s belongings or valuables brought in the massage session. Appointments are not transferable. The therapists providing the therapy services are not qualified to diagnose, prescribe prescription medications any physical or mental illness. In event of any unforeseen complications to the health of the clientele, that may arise if the clientele already suffers from any of the ailments and conditions, Pure Med Therapy and its employees, officers, management and directors, shall not be held liable or responsible. Patients are responsible for their own self care such as properly eating and hydrating before, during, after a session. Also the management shall not be responsible or held liable for any costs, expenses, loss, or claims that may arise from or in relation to any medical assistance or treatment required by the member. Any or all such costs, expenses, loss or claims shall be the total and full responsibility of the member. Pure Med Therapy reserves the right to refuse service to any client for improper conduct. Inappropriate conduct or language during a service will result in the immediate termination of the service. We reserve the right to define inappropriate conduct. Pure Med Therapy reserves the right to change these terms and conditions from time to time, in its sole discretion. It is strictly the client and/or client’s caregiver’s responsibility to alert Pure Med Therapy of any allergies and to protect the client from any relevant allergies. It is strictly the client and/or client’s caregiver’s responsibility to alert Pure Med Therapy of any healthcare diagnoses and to follow the recommended treatment plan by the client’s primary care physician. By signing this form, the client revokes the right to sue Pure Med Therapy for any reason.
